One of the most common questions patients asked me was this:
Is drinking diet soda safe?
For years, we’ve been told that diet soda is a smart compromise. No calories. No sugar. Just sweetness without consequences.
But the real question isn’t whether artificial sweeteners cause immediate harm.
The real question is: What happens when you regularly consume chemicals that interact with your brain, pancreas, and gut—every single day?
The Insulin Trick
When something sweet touches your tongue, your brain assumes sugar is coming. Signals go to the pancreas, and insulin is released in preparation.
But when you consume artificial sweeteners, the sugar never arrives.
Now you have insulin circulating with no glucose to manage. That mismatch may increase hunger and drive cravings. Instead of helping with weight control, it may subtly push you to eat more.
Zero calories does not necessarily mean zero metabolic effect.
How Sweeteners Work
Artificial sweeteners fall into two categories:
Nutritive sweeteners (xylitol, sorbitol) contain some calories and may prevent cavities—but often cause digestive upset.
Non-nutritive sweeteners (aspartame, sucralose, saccharin) contain no calories but still trigger sweet receptors intensely.
Plant-derived options like stevia and monk fruit are plant-based in origin, but they’re still concentrated extracts that condition your palate toward intense sweetness.
If you choose to use a sweetener occasionally, small amounts of raw honey, maple syrup, or blackstrap molasses are less disruptive. But they’re still added sugars—best used sparingly within a whole-food dietary pattern.
Gut Health and Inflammation
In previous articles, I’ve discussed the importance of whole foods to promote digestive health.
Artificial sweeteners contain no fiber and do not nourish the gut microbiome. Research suggests they may negatively alter gut bacteria. When the microbiome becomes imbalanced, intestinal permeability—often called “leaky gut”—may increase.
This allows inflammatory compounds into the bloodstream, which may promote fat storage, insulin resistance, and metabolic syndrome.
So if we care about long-term health, we can’t ignore the gut.
Cancer and Long-Term Concerns
A large French study following over 100,000 people found that higher intake of artificial sweeteners—particularly aspartame and acesulfame-K—was associated with a 13% increased cancer risk.
Does that prove causation? No. Observational studies show correlation, not cause.
But here’s what we know: the evidence on long-term safety is incomplete. We’re still learning how these chemicals interact with human biology over decades.
So the question becomes: Why expose yourself daily to something with uncertain risks when water, black/green tea, or herbal teas will quench your thirst?
The Brain and Sweetness Conditioning
Artificial sweeteners can be hundreds of times sweeter than sugar. That intensity strongly stimulates dopamine pathways in the brain.
The result? Greater cravings for sweetness.
Instead of training your palate to appreciate the natural sweetness of fruit, you reinforce a preference for hyper-sweet flavors.
This is not freedom from sugar. It is dependency in a different form.
What About Sugar?
Here is where nuance matters.
Sugar is not automatically the enemy. The problem is context.
In a diet high in fat and processed foods, excess sugar contributes to weight gain and metabolic dysfunction. But when people eat a whole-food, plant-based diet centered on starches, legumes, vegetables, and fruit, the body handles natural sugars differently.
Why? Because whole fruit comes with fiber, water, and phytonutrients. That fiber slows sugar absorption and prevents blood sugar spikes.
A banana is not a can of Coke.
Artificial sweeteners offer none of those protective elements. They deliver intense sweetness without nutrition—training your palate to crave what whole foods cannot provide.
The Honest Bottom Line
Artificial sweeteners may be less harmful than drinking sugary soda all day—especially within a high-fat, highly processed diet.
But they are not better than drinking safe beverages like water or tea.
They may influence insulin signaling, alter the gut microbiome, increase cravings, and carry long-term risks we do not yet fully understand.
Health is not about clever substitutions or shortcuts.
It is about returning to what is natural and simple.

Every time you hear someone telling you something about artificial sweeteners or saturated fat, ask them how much sugar or carbohydrates sits in one cup of coke, plus a cup of orange juice you drank earlier that day, plus a ‘healthy’ ‘natural’ sweetened smoothie you will drink later in the day. You will be surprised that you drank about full 8oz cup of sugar in just these three drinks, now add a bagel or even a ‘healthy’ corn muffin, and you just ate another 5 spoons of what would equal sugar, just goes by another name.
Bottom line we have an epidemic of diabetes in the USA and its not from artificial sweeteners. Diabetes is the precursor to almost every disease you can imagine, just ask someone who has it, and they will tell you how bad it is. Artificial sweeteners is like the e-cig of cigarettes, its the healthier of the two options.
These talking points are usually compiled by health institutions that ran studies occasionally sponsored by sugar companies, because these talking points do not push people to drink water when they reach the soda machine, they just will go for the most dangerous of the two, sugar.
You keep lumping fats (solid at room temperature, naturally saturated, ie. butter, tallow, shmaltz, lard, coconut oil, palm fruit oil) and seed oils into one heading of fat. This is simply disingenuous and misleading. Your line should be corrected to “In a diet high in seed oils (aka “vegetable” oil and other industrial lubricants) and processed foods…”
