The Lakewood Board of Education will again be distributing free food boxes for all children under the age of 18 on Wednesday. However, to receive a food box, you will now need to bring with you the form posted below.
The form is intended to allow organizers to anticipate the number of meals they will need to distribute, and thereby lessen the wait time for those arriving for food boxes.
Please print and fill the form prior to arrival for the food boxes.

Dont understand how to fill this out. If i have 3 kids in the cheder which is open and they receive free lunches, am i eligible. i also have a child in highschool. nobody learns remotely. please give instructions here
Fill out column A with the names of all your children under the age of 18.
Fill out column B with their ages.
In column C, for each child, if they are not receiving any school meals (for example, a baby that is home), fill out a 1.
In the bottom box, fill in the square with the sum of the 1s in that column.
Calculate that number by 7 and fill that into the circle.
In column D, for each child, if they are receiving school lunches, fill in the number of days per week they are NOT receiving lunch from school.
In the bottom box, fill in the sum of all the numbers in column D.
In the bottom-right-most box on the chart, fill in the value that is in the circle on the bottom of column C, and fill in the value that is in the circle in the bottom of column D.
Then add those two numbers together.
This sum is the amount of meals you are “zoche” to from the BOE. This is the number that they need to know.
For your 3 kids in The Cheder, you would probably fill in the number 2 for each of them in column D, because they do not receive lunch on Friday and Shabbos. (Unless they come home with a “Shabbos bag”, in which case you should fill in a 0.)

Are you serious? Why does anyone HAVE to have a printer?
If someone in general does not need a printer, and only needs to print things every once it a while, it does not make sense for them to buy one.
To comment number 8: ARE YOU SERIOUS? Respectfully, YOU DO NEED A PRINTER SO YOU CAN COMPLY WITH THE FOOD FORM REQUIREMENTS SO YOU CAN CLAIM YOUR FOOD BOX. You actually know the answer if you don’t have a printer. Go to someone who does. Hashem should be with you.
If my son is staying in his dorm (in NJ) but I pay for food because they don’t have a lunch program, do I count him as 7?
The other kids in Lakewood schools get counted as 2 each? (Because they don’t get lunch on shabbos and Sunday)
The total is how many meals they aren’t getting? That’s a lot more meals than I’ve been taking.
Does anyone know?
